Patient Information

Before your first appointment and once a year thereafter, you will need to complete or update your chart information. If you would like, you may print it from the button on the right, fill it out, and bring it with you to your appointment.

When you make an appointment we will now send a link to fill out your forms electronically, if that suits you better. Also, we have an app for that!, and can send the Modento app invitation for iPhone or Android.

What to Expect:

1st Visit - Come in 15 minutes early to fill out paperwork. If you have dental insurance, we will contact the insurance company at this time to outline your benefits.

Procedures - Each dental procedure is explained and a written treatment plan is given. Patients are encouraged to discuss treatment options and ask questions.

Cleanings - Twice a year you should have your teeth professionally cleaned. The dental hygienist will take full-mouth X-rays, clean, polish, floss, and examine each of your teeth. The dentist will then re-examine your mouth and perform a visual oral cancer screening of your tissues. A comprehensive written treatment plan--including pricing--is created to address all issues noted.
